Financial Policy

Our consultation fee is $100. This fee is non-refundable and is due at the time of scheduling the appointment. This fee will be applied towards the cost of the procedure.

A non-refundable deposit of $500.00 is required at the time of scheduling your surgery. Deposits are non-refundable for all circumstances. The remaining balance is due at your pre-operative appointment (usually scheduled 3-4 weeks before surgery). If you RESCHEDULE your surgery for any reason following full payment, an additional rescheduling charge of $500 will be incurred which is not included in the fees originally quoted. If you CANCEL your surgery after your pre-operative visit and/or full payment, we reserve the right to retain 50% of all fees paid plus an additional $250.00 for administrative costs.

FORMS OF PAYMENT ACCEPTED: Cashier’s checks, money orders, all major credit/debit cards, and care credit. WE DO NOT ACCEPT CASH OR PERSONAL CHECKS.

The fees quoted are unique to each individual and may not apply to other individuals. The quote will be honored for 6 months from the date of the consultation.

Should future revisional surgery be required or requested, additional charges will be incurred which are not included in the original fees quoted.

Your quote includes all pre- and post-operative visits for private pay surgery. However, you may incur additional expenses with your surgery including, but not limited to, pre-operative laboratory work, prescription medications, mammogram, EKG, and pathology.

Private pay surgery is not billable to any insurance carrier and billing information is not generated to assist you in collecting from a third party.

Patients who are planning to file disability should be aware that there is a $30 charge to complete these forms and will take 3-5 days to complete. It is the responsibility of the patient to provide those forms to our office for completion. In general, the time off allotted is usually between 1-4 weeks depending on your type of work and the procedure you are having done. We cannot guarantee approval as all disability must meet your employer’s qualifications and requirements.

Once the patient is out of surgery, the caregiver is expected to be present for post-operative instructions. If he/she is not present and/or late by over 30 minutes, additional charges will be applied at $50/hour to pay for additional nursing care.

In general, payments will be based on the amount of the procedure, and the length of time for repayment. Some patients may require a co-signer in order to obtain the loan.
